Every family begins the wish-granting process by meeting with their wish-granting volunteer team. On their first visit, your volunteers will bring the Wish Packet and Liability Form which they will fill out with you. These forms provide us with information about your child and your family and provides you with some info about the wish-granting process. 

There is no need to fill out these forms before your wish-granting volunteers arrive. They will bring copies. We are providing them to you here so that you can get a sense of what kind of information we will be asking for.
