Every family begins the wish-granting process by meeting with their wish-granting volunteer team, what we call a Wish Team. On their first visit, your Wish Team will bring the Wish Packet and Liability Form, which they will fill out with you. These forms provide us with information about your child and your family and provides you with some info about the wish-granting process. 

There are additional forms that you may need to complete, depending on your specific family situation:

• Absent Parent Form 
• Single Parent Form 
• Communication Profile for Non-Verbal Children

There is no need to fill out these forms before your Wish Team arrive. They will bring copies. We are providing them to you here so that you can get a sense of what kind of information we will be asking for.
