Wish Family Required Paperwork

Each parent or legal guardian will need to complete and sign a copy of the Wish Family Packet. Upon receipt of wish ideas, your child’s wish coordinator will send a digital version of this paperwork through DocuSign.
Depending on your child’s chosen wish type, there may be additional forms to complete. For example, our partners at Walt Disney World and Give Kids the World have their own forms they ask wish families to complete.Â
For some wish types, Make-A-Wish staff will reach out to your child’s doctors for approval before committing to the wish to make sure the wish is medically safe and appropriate. We will take care of this while you complete other forms.
Once we have all paperwork required for your child’s wish type and medical approval if applicable, the wish has been officially discovered and it is ready to move forward to Wish Creation.
