Process And Forms For Parents/Guardians
Your child has qualified to receive a magical wish experience, and Make-A-Wish Georgia is honored to be part of this journey. Granting your child’s most cherished wish is a true honor. We look forward to working with your family during the entire wish granting process.
By now, you have received your initial welcome call. We hope we have answered your questions, but please reach out to us any time if you think of more. As explained on the call, your child’s wish is one of 1,100 that we are diligently working to grant in Georgia. The next step is for your child’s Volunteer Wish Granters to be assigned. We ask that you start now helping your child brainstorm as many wish ideas as they can. Make a list of those ideas! We’ll be excited to talk through each one at the Wish Discovery Visit.
Once we have identified a Wish Team of two trained Volunteer Wish Granters, we’ll let you know their names so you can be on the lookout for their phone call. The Volunteer Wish Granters will contact you to schedule your VIRTUAL wish discovery visit. This process can take 2-3 months.
Volunteer Wish Granters will meet with your family VIRTUALLY to complete paperwork and speak with your child about their wish ideas. The Volunteer Wish Granters will fill out all the paperwork on your behalf. We have also provided links to this paperwork below in case you would like to review these forms ahead of time. Again, there’s no need for you to complete them yet. Your Volunteer Wish Granters will take care of that for you at the Wish Discovery Visit.

Once the Wish Discovery Visit is complete, your Volunteer Wish Granters will return all the wish paperwork to the Make-A-Wish Georgia office for review for our staff. You will be contacted by a staff member to further discuss any questions and review the next steps in the approval process. Once the wish is medically and internally approved, a Wish Coordinator will be assigned to work with your family to plan and grant your child’s wish!
Through the wish granting process, we would like to collect the following from your family. Please begin gathering these documents now and send them our way
- Copy of a photo IDs or driver’s license for parents/legal guardians
- Copy of the birth certificate for your wish child
- Copy of the birth certificate for any siblings under the age of 18 who reside in the home
- Copy of a photo IDs or driver’s license for any sibling 18 years old or older who reside in the home
- Communication Profile for Non-Verbal Children (if applicable)
